Services are provided to improve socialization and other non-medical support to individuals in their home. The focus is on assistance with housekeeping, preparation of meals, shopping, and errands. Homemaker services enable the participant or the family member(s) or friend(s) with whom the participant resides to maintain their primary private home. Homemaker Services include cleaning, laundry, meal preparation, and other general household care including but not limited to vacuuming, sweeping dusting, and mopping.
Services are provide service that is primarily emotional support and companionship for individuals who want to remain independent at home. This service is intended to assist an individual who is age 18 or older to participate more meaningfully in home and community life. It is provided to individuals living in private homes for the purposes of providing supervision or assistance that is designed to ensure the individual’s health, safety, and welfare or to perform activities of daily living for the individual.
Services are deliver these services in the Individual’s home and/or community. This will consist of services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. Services consist of support in the general areas of self-care or assistance with activities of daily living (ADLs), communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources.
Community Features residenial group homes provide services based on the service level of needs to support individuals to achieve their habilitation outcomes (goals). Community Features utilizes the participant's individual’s ISP to provide services and staffing support that our person-centered. The support addresses the current needs of the individual and allows for improvement in many of the areas necessary to live successfully in the community. This includes ensuring assistance, support, and guidance (which includes prompting, instruction, modeling, and reinforcement) will be provided as needed to enable the participant to: Carry out activities of daily living such as personal grooming and hygiene, dressing, making meals and maintaining a clean environment. As well as instrumental daily living activities such as support employment, shopping, community integration etc.…Residential habilitation services are provided in the home 24hrs/7 days a week.
Community Features admissions, provisions of services, and referrals of individuals shall be made without regard to race, color, religious creed, disability, ancestry, national origin (including limited English proficiency), age, or sex.
